Paperwork Needed:

When you file a long term disability claim, the process will go more smoothly if you have the right paperwork in hand. Here are three important things you will need: 

  • Medical records. Hospitals and physicians keep files on all of their patients. These files include information on how and when you were diagnosed and treated, test results, and prescriptions for medications. You have rights to copies of these records – ask for them. Having them will help in many cases.
  • The more detail you have, the better. If there are parts of your records that you don’t understand, ask your doctor about them. If something is wrong or seems inaccurate, ask your doctor to make the necessary changes or supply any missing information. 
  • A copy of your insurance policy. You should have a copy of the original long term disability policy as well as any updates to the contract. If you don’t have a copy of your policies, ask your insurance agent or your employer to send them to you.

Filling Out the Claim:

Most long term disability claim forms require statements from you, your doctor, and your employer. When giving statement, remember to be thorough, and be careful. The information you provide on these forms will determine whether the insurance company approves your claim. Try and stay on top of filing your forms. If you wait too long, you might lose your right to seek benefits. Don’t wait until the last minute.

Submit your claim as soon as you can. Most policies require that a claim be filed within a certain amount of time (the Statute of Limitations) — often 60 days — after the patient is unable to work. If you wait too long, you might lose your right to seek benefits. Act fast.

No matter how prepared you are, the long term disability insurance company is very likely to ask for additional information — if only to delay the process. At this point, you should also be keeping track of all contact you have with the insurance company — so make notes of any calls, and keep copies of all notices you receive by mail. In the notes include the date, time, and the details of what was discussed. If your claim is denied, send a letter back stating that you disagree with the decision — and keep a copy for your own records.
